PAYMENT POLICY
Medical & Psychiatric services are provided and billed by Michael Lesem, MD PA. Dr. Michael Lesem participates in a few of the major health insurance plans in Texas and is currently working on several others. Below are the health plans which we currently accept.
If your plan is not listed, please call the office to see if we accepting the plan or if your health plan has contracted with another company with which we are in-network. If Dr. Lesem is out-of-network and you still want to see him we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.
Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

What billing or insurance information will I receive?
You will receive a copy of your bill. After your insurance has paid, we'll provide you with a monthly statement of your account.
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.
I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?
If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
Definitions

Co-Pay - Amount that the patient is required to pay at the time of the visit. For Example, A Co-Pay of $30 means that the patient pays $30 at each visit.

Deductible - Amount that the paient has to pay each year before their Co-Insurance come into effect. For Example, if the patient's deductible is $1,500 for the year, then they would pay the office visit (contracted amount) to the provider until that amount is reached. When it is reached. if the patient's Co-Insurance is 20%, then the patient would pay 20% of the contracted value.
